Simply go to THIS PAGE and fill out your information. It’s very simple to do and will help us keep track of everything.
It’s a Flash based form – so you’ll need Flash installed on your machine. Some people in offices and government networks may experience problems and should fill the form out at home.

That being said – we are no longer conducting initial interviews on the telephone. We’ll reserve telephone conversations for times when it is most needed rather that spending hours talking to each person seeking an evaluation.
We just don’t have the time.
